Nomination Form - Outstanding Volunteer

Rules and eligibility:

• The application for all outstanding volunteers must be received by March 10^, 2024.
• The nominee can still be active as a volunteer or was active in the past.
• A nominee may be nominated more than once, but may not win the Volunteer of the Year Award more than once in five years.
• City staff and serving members of council shall not be nominated for this award.

Award Ceremony:

• The Outstanding Volunteer Brunch award ceremony shall take place on Sunday, April 14^, 2024, the first day of the National Volunteer Week.
• The ceremony will be a brunch in one of the Township’s hall.
• Recipients of all the awards will be noticed before the event.
• The recipient for the “life-long achievement award” will be invited to the award ceremony with six extra tickets.
• Recipient will be invited to the Outstanding Volunteer Brunch award ceremony with three extra tickets.
• All nominees will be invited to the Outstanding Volunteer Brunch award ceremony with one extra ticket.

All rules and regulation are based on the Volunteer recognition policy. The nominator and nominee must consent for the Township of Alfred and Plantagenet to collect, share and disclose all information related to the nomination.
